Why stomach sleepers get back pain
Face-down, the heaviest part of your body, the pelvis, sits right over the softest part of a typical mattress. As it sinks, the lower back arches. At the same time, your head is turned to one side for hours, which twists the neck. Neither is a disaster for one night; both add up over months.
You can reduce the arch with a firmer bed, and you can reduce it further with a thin pillow under the hips. Some stomach sleepers also do better with a very flat head pillow or none at all. If you can train yourself toward side sleeping, that helps too, but most people find it hard to change a lifelong habit.
