What is the best mattress for back pain?
The short answer: For most people with everyday back pain, a medium-firm bed (about 6–7 out of 10) is the place to start; that is what the best-known trial on the subject favored. On Amazon right now the DreamCloud Premier Hybrid is the safest all-round pick, the Helix Midnight Luxe is the one for side sleepers whose lower back aches, the Nectar Premier is the memory-foam choice, and the Zinus Green Tea 12-inch is the budget way to test firmer support. Stomach sleepers and heavier bodies need firmer still.

How this list was put together
Back pain and mattresses have been studied less than marketing suggests, but the evidence that exists points one way. In a 2003 randomized trial published in The Lancet, Kovacs and colleagues gave 313 adults with chronic low back pain either a firm or a medium-firm mattress for 90 days; the medium-firm group reported less pain in bed and on getting up. Later reviews, including Radwan and colleagues in Sleep Health (2015) and Caggiari and colleagues in the Journal of Orthopaedics and Traumatology (2021), landed in the same place: medium-firm tends to beat both very soft and very hard for most people. That is our starting point, not a law. Stomach sleepers and heavier bodies usually need firmer; light side sleepers usually need softer.
From there we only did things a careful buyer could check for themselves. We read each Amazon listing line by line, compared the maker's own firmness wording, looked for zoning claims, wrote down trial and warranty terms exactly as listed, and noted who actually sells the bed. Where a listing said nothing, the table below says so. We did not sleep on these mattresses and we have not invented scores.
The 12 best mattresses for back pain, one by one
DreamCloud Premier Hybrid (Queen)
A medium-firm hybrid with a quilted top that suits back sleepers and people who change position.
Why it is here: Medium-firm, coils for support, two inches of memory foam for the shoulders, sold by Amazon.com, and the longest trial in our set. It fits the most common back-pain sleeper: someone who starts on their back and ends up on their side.

You sleep on your back or switch positions, want coils for bounce and airflow, and like the idea of a full year to decide.Look elsewhere if
You are a lighter side sleeper who wants to sink in at the shoulder, or you need a bed that is truly firm.
Helix Midnight Luxe (Queen)
The bed many other back-pain lists rank first, now sold on Amazon: a 6-out-of-10 hybrid with a zoned lumbar section and a soft Euro top.
Why it is here: A 6-out-of-10 hybrid with a zoned lumbar section in the coils and a soft Euro top: cushion for the shoulder and hip, support under the waist. It is the bed several of the best-known back-pain lists put first. The catch is that its listing states the trial two different ways, so confirm it before you order.

Nectar Premier Memory Foam (13 in) (Queen)
Nectar's step-up foam bed: thicker memory foam and a cooler cover than the Classic, rated 6.5 out of 10 by the maker.
Why it is here: Nectar prints 6.5 out of 10, the middle of the medium-firm range the research favors, and adds an inch of memory foam over the Classic. Sold by Amazon.com with a 365-night trial, for well under the price of the hybrids around it.

Best mattress for a bad back: match it to where it hurts
Back pain is not one thing, and a list that treats it as one thing will steer some readers wrong. If the ache sits low, just above the belt, look at beds with zoned or firmer support under the waist: our best mattress for lower back pain page goes deeper. If it is between the shoulder blades or into the neck, the problem is usually a shoulder that cannot sink in, which is why our upper back pain picks run softer. Pain that travels down one leg has its own page, mattresses for sciatica, and so does the combination of back and hip pain.
How you sleep matters as much as where it hurts. Side sleepers need give at the shoulder and hip; back sleepers need the lower back filled in; stomach sleepers need firm. Body weight shifts all of it: heavier sleepers usually need one step firmer and a stronger coil unit.
Why so many top-ten lists look alike
Read five of the best-known 2026 back-pain rankings and you will see the same names rotate through first place: a handful of online-only brands, each with a discount code under the buy button. A few patterns are worth knowing before you trust any of them, including this one.
The winners are often sold only on the maker's own website, so you cannot see what other buyers paid, who they bought from or what went wrong for them. Test counts are large and unverifiable: a headline number of beds "tested", with no data published to back it. Sale banners go stale; in late October we still found summer and Labor Day promotions sitting on several pages dated 2026. And almost none of them mention that a topper, a knee pillow or a change of sleep position is often the cheaper first step.
Some of those winners are on Amazon if you look: the Helix Midnight Luxe and the Nectar Premier both are, and both are ranked on this page. For everything else we built the list the other way round. Every bed above is on Amazon from its maker or from Amazon itself, so the price, the seller and thousands of owner comments are one click away. When a well-known bed is missing, we say why.
Cheaper back pain relief to try before a new mattress
If your current mattress is under about seven years old and has no visible dip, try firmness before you replace anything. A topper changes the feel for a fraction of the cost: soft foam if your shoulder and hip ache on a hard bed, a firm two-layer topper if you sink on a soft one. Our topper guide explains which way to go. Side sleepers often get real relief from a pillow between the knees, and back sleepers from a thin pillow under them. An adjustable base that raises the knees slightly takes load off the lower back in a way no mattress can.

What the research does not say
No study shows that a particular brand cures back pain, and no mattress has been proven to fix a disc problem or nerve compression. The trials compare broad firmness levels, mostly in people with chronic, non-specific low back pain. Several listings in this guide cite studies or endorsements we could not find published; where that happens, we mention it on the product page and leave it out of our reasoning. If your pain is new, severe or comes with numbness or weakness, a doctor's visit will tell you more than any mattress page. The research behind this list is laid out in what doctors and studies actually say.
Back pain and mattresses: reader questions
What is the best mattress for back pain?
For most people with ordinary, long-running back pain, a medium-firm mattress around 6–7 out of 10 is the best starting point. On Amazon today that points to the DreamCloud Premier Hybrid for most sleepers, the Helix Midnight Luxe for side sleepers whose lower back aches, the Nectar Premier if you prefer memory foam, and the Zinus Green Tea 12-inch on a budget. Stomach sleepers and heavier people usually need firmer.
Is a firm mattress better for a bad back?
Not usually. The best-known trial found medium-firm beds beat firm ones for people with chronic low back pain. Very firm beds suit stomach sleepers and some heavier sleepers. If you sleep on your side, a firm bed often moves the pain to your shoulder and hip. See firm vs medium-firm.
Which top-rated mattresses for back pain can you buy on Amazon?
Every mattress on this page is sold on Amazon by its maker's own storefront or by Amazon.com, including DreamCloud, Helix, Nectar, Casper, Brooklyn Bedding, Leesa, Zinus and Avenco models. We do not show star ratings because they shift daily; the 'Read owner reviews' button opens the current ones.
Memory foam or hybrid for back pain?
Both can work. Hybrids give more bounce, better edges and run cooler, which helps heavier sleepers and couples. Memory foam isolates motion and contours more closely. The firmness level matters more than the material. Our memory foam vs hybrid comparison goes through it.
How long does it take to know if a new mattress helps?
Give it three to four weeks. Your body takes time to adjust, and several makers say so outright; Serta's iComfort listing suggests up to 60 days. That is why trial length matters: a 100-night trial leaves time to decide, a 30-night window does not.
Can a bad mattress cause back pain?
It can make it worse. A bed that sags lets the hips drop below the shoulders, and one that is too hard for your build can push the spine out of line on your side. A visible dip, waking stiffer than you went to bed, and sleeping better away from home are all signs to look at the mattress.
When should I see a doctor instead of buying a mattress?
If pain follows a fall or injury, spreads down a leg, comes with numbness, weakness, fever, unexplained weight loss or bladder or bowel changes, or keeps you awake whatever position you try. Those are medical questions first.
Is the Helix Midnight Luxe sold on Amazon?
Yes. At our October 30 check it was listed new by 3Z Brands, which the listing names as the manufacturer, from Twin XL to California King, with either a TENCEL pillow top or a GlacioTex cooling cover. Its listing gives the trial as 365 nights in one place and 120 in another, so confirm with the seller before you order. Our Helix Midnight Luxe review has the details.
Nectar Premier or Nectar Classic for back pain?
The Premier is the softer, thicker one: 13 inches with an extra inch of memory foam, rated 6.5 out of 10 by Nectar, which suits back and combination sleepers. The Classic we rank is the firm version, better for stomach sleepers and people who sink into everything. Both are sold by Amazon.com with a 365-night trial. Full comparison: Nectar Classic vs Premier.
Neutral Spine is not a medical provider. A mattress can make sleep more comfortable; it does not treat a spinal condition. Pain that follows an injury, spreads down a leg, or comes with numbness, weakness or fever needs a doctor first. Full disclaimer.
