Who actually needs a firm mattress
There is a long-standing belief that a bad back needs a hard bed. The research does not support it as a blanket rule: in the best-known trial, medium-firm beat firm for chronic low back pain. But "medium-firm suits most people" leaves plenty of people for whom firm is right:
- Stomach sleepers, because a soft bed lets the pelvis sink and arches the lower back.
- Heavier sleepers, because they compress a bed further; their medium-firm is someone else's firm.
- Back sleepers who wake with a sagging feeling, especially on an older or softer bed.
- People who have tested it. If you sleep better on a firm hotel bed or on the floor, that is real information.
If you sleep on your side, be careful. A firm bed pushes back hardest at the shoulder and hip. Read firm vs medium-firm before buying.

Firm, extra firm, and what the words mean
Makers do not share a scale. In this list, "firm" runs from the Brooklyn Bedding Signature Firm, which most people would call firm-ish, to the Plank's extra-firm side, which its maker rates a ten. Where a maker gives a number we show it on the card; otherwise we translate the word and say so. The firmness guide has the full scale.
Test firm before you commit

Is a firm mattress good for back pain?
For stomach sleepers and heavier sleepers, often yes. For most people with chronic low back pain, research favors medium-firm. Side sleepers usually do worse on firm.
What is the firmest mattress on Amazon?

Can a mattress be too firm for back pain?
Yes. If you wake with sore shoulders or hips, or feel a gap under your lower back when lying flat, it may be too firm for you.
How long does it take to get used to a firm mattress?
Usually two to four weeks. If you are still sore after a month, it is probably wrong for you; that is what a trial is for.
