The short answer: An orthopedic mattress is just a supportive, medium-firm to firm bed, often with zoned support. The Casper Original Hybrid is the best ortho-style pick for most people, the Sealy Posturepedic Pro Haverford is the classic firm hybrid, the Serta iSeries NXG 3500 adds five-zone coils, and the BedStory 14-inch and FP Flexpedic 14-inch are budget beds marketed for spinal support.

What "orthopedic" means on a mattress
Nothing official. There is no standard a mattress has to meet to be called orthopedic, ortho, or "medical" in ordinary retail, and the word is used on beds that range from very firm innersprings to soft foam. When people search for an orthopedic mattress, they usually want three things:
- Support that keeps the spine in a neutral line, without the hips sinking.
- Firmness in the medium-firm to firm range.
- Often, zoning: firmer under the waist, softer at the shoulders.
So rather than trust the label, check those three things on the listing. Every card below shows the maker's firmness wording and any zoning claim.
What about a "medical mattress"?
In a clinical setting, a medical mattress usually means a pressure-redistribution mattress for people who spend long hours in bed and are at risk of pressure injuries. Those are prescribed and specified by care teams and are a different category from the beds on this page. If you or a family member needs one, ask the care team or an occupational therapist.

Before you order: reader questions
What is an orthopedic mattress?
A marketing term for a supportive mattress, usually medium-firm to firm and sometimes zoned. There is no official standard behind it.
Is an orthopedic mattress good for back pain?
A supportive, medium-firm bed is good for most people with back pain, whatever it is called. Check the firmness and support, not the label.
What is the difference between an ortho mattress and a regular one?
Often nothing beyond the name. Some 'ortho' beds are simply firmer; some have zoned support. Read the build details.
Is a medical mattress the same as an orthopedic mattress?
Not usually. In healthcare, a medical mattress normally means a pressure-redistribution mattress used for people at risk of pressure injuries.
