The short answer: A topper works when your mattress is sound but the wrong firmness. A new mattress is needed when the bed sags, is older than about eight years, or you have tried a topper and still wake sore. Our picks: TEMPUR-Adapt Supreme topper and DreamCloud Premier mattress.

TEMPUR-Adapt Supreme Topper (3 in)
The firmer of the two Tempur toppers; adds pressure relief without making the bed mushy.

The test
Strip the bed and look along the surface with a straight edge, such as a broom handle laid across it. A dip of more than an inch or so where you sleep means the support has gone. No topper fixes that. If the surface is level but the bed feels too hard or too soft, a topper is a reasonable first step.
A topper is enough if
- The mattress is less than about seven or eight years old.
- There is no visible dip.
- The problem is feel: too hard at the shoulder, or too soft under the hips.
Replace the mattress if
- There is a dip, or you roll toward the middle.
- It is old, or the warranty has run out.
- You tried a topper for a few weeks and still wake sore.

Can a mattress topper fix back pain?
It can help if the problem is firmness on a sound mattress. It cannot fix sagging.
How long does a mattress topper last?
Typically a few years. Dense foam lasts longer than cheap, light foam.
